Home care nursing (Domiciliary Care)
The
majority of patients are cared for in their own homes where the family
becomes the sole unit of care. An experienced, specially-trained nursing
sister assesses the patient and makes a commitment to be accessible
whenever the need may arise, visiting the patient on a regular basis at
home and providing physical care, practical help and emotional support.
In-patient care
Sungardens Hospice is
privileged to have an In-patient Unit as state hospitals no longer care for
patients once treatment has ceased. The unit supports the home care
service. Patients are admitted for a short period intensive medical care,
including pain and other symptom control, or for those who wish to die
away from home, or in a hospital ward. Patients are also admitted for
up to 3 weeks for respite when the families need to have a break.
The
aim of the I.P.U. is to provide a homely atmosphere where each patient
receives holistic care according to his/her needs, from skilled
palliative care from Doctors, sisters, nurses and caregivers. Many of
our patients return home once symptoms are controlled. While patients
are in I.P.U., the families are free to visit whenever they wish and
are allowed to be involved with the care of their loved ones and at the
same time receive the support they need.
Day care in Mamelodi
Patients
come to the hospice day care centre once a week to enjoy a social
outing and to be medically assessed. They take part in various activities
such as artwork, sewing or gardening. They listen to music, socialise
with other patients and enjoy a cooked meal.
Bereavement Counselling
We
provide a bereavement service, maintaining contact with the family for
as long as needed after the death of a patient. Grief is a private and
individual process and we try to help families in whichever way they
need.
Hospice Team
Hospice offers a
programme of specialised home-based palliative care to terminally ill
patients and support to their families and provide this support to
persons in the last phases of uncurable disease, making sure that their
lives remain as meaningful and comfortable as possible
Referrals
Referrals are accepted from family members, with the patient’s consent, or from a medical source.
